Curricular Practical Training (CPT) Request

This form is to request a Curricular Practical Training (CPT) Internship Authorized I-20. Note that if you have any holds on your student account, we may not be able to assist you until all holds are removed.

Before requesting a CPT Internship Authorized I-20, review the criteria listed below:

• Present in the U.S. in valid F-1 status at the time of application.
• If the internship is required by the academic program, students may engage in the internship as early as their initial quarter. Otherwise, students must be enrolled on a full-time basis for three consecutive quarters (one academic year) at a DHS-approved school without gaps in enrollment such as Post-Completion OPT. We may ask for an unofficial transcript from the previous institution. 
• Registered for a credit-bearing internship class.
• Have an internship offer letter.
