On this page you will find two presentations related to submitting the required documents for practicum/clinical fieldwork. The first details the actual requirements, such as the kinds of medical records to submit, while the second demonstrates how to navigate the CastleBranch website, in particular the Clinical Requirements Tracker.
You will need to log in with your UW NetID to view both presentations.
As a reminder, the UW as an institution requires immunity verification for all students, which is separate and in addition to the School of Nursing and Healthcare Leadership clinical requirements. Go to https://www.tacoma.uw.edu/immunization to learn more about those requirements.
Also, visit the UW Tacoma Student Health Services page to learn about the free or discounted vaccines and testing you can receive as a student. This may be a faster or cheaper alternative to making an appointment with your primary care provider.
