SNHCL Outstanding Alumni Award Nomination form

The UW Tacoma School of Nursing and Healthcare Leadership's Outstanding Alumni Award recognizes an alumnus whose career in healthcare exemplifies excellence in clinical practice, education and/or leadership. This award is based on the individual’s record of achievement.

Criteria include:

1. The nominee must be an alumnus, a former student who earned a Bachelor of Science in Nursing, Master of Nursing or Bachelor of Arts with a major in Healthcare Leadership degree.
2. The nominee has demonstrated outstanding accomplishments and contribution to Nursing and/or healthcare in leadership, education, or practice roles.
3. The nominee’s selection will bring recognition to the School of Nursing and Healthcare Leadership and University of Washington Tacoma.

Nomination Process:

1. Nominations are submitted by faculty using this webform. Nominations are due by the last Monday of April each year.
2. Complete all fields below, which includes uploading a nomination letter. Additional documents may be included for support of the nomination.
3. Faculty vote on the recipient for the award and forward their recommendation to the Dean for action.
