Please fill out this form to request a STEM Extension Optional Practical Training (OPT) recommended I-20. You will need to have your most recent I-20 ready to fill out this form!
Please note that if you have any holds on your student account, we may not be able to assist you until all holds are removed.
Before requesting a STEM Extension OPT I-20, please read the STEM Extension OPT information carefully and note the following:

STEM Extension OPT is available only for students who completed a major in the Science, Technology, Engineering or Math field. -
Students must apply BEFORE their current EAD expiration date. -
Students must have a job or an offer from an E-verify employer at the time of application. -
Students must upload completed and signed Form I-983 on this request. The file must be in jpg or pdf format and cannot exceed 2MB. -
Students on the OPT benefit must notify ISSS within 10 days of changes related to address and employment information. To report your employment information update, please access the Employment Info Update Request during Post Completion and STEM Extension OPT webform. During post-completion and STEM extension OPT, students can report address updates in SEVP Portal and in MyUW.
