The Academic Alert Referral form is used to identify students who may be struggling with academic difficulty or at risk of failing. Faculty and staff should use this form to alert the academic advisers of any student who is not making satisfactory progress or is exhibiting behaviors that may lead to academic difficulty. Once a student is identified, the academic advisers and other appropriate staff will work with the student to improve his or her academic opportunity for success.
Request for Use of Course Fee Funds
Request the use of course fees to pay for course-related costs such as field trips, course related supplies and equipment. Effective September 30, 2017 all funding request for field trips and approved course supplies must be received one quarter prior to your teaching the CORE class for which you are requesting funds. The cut off for funding request will be the last day of finals every quarter. Requests received two quarters in advance will be given priority. Again, all requests must be submitted a quarter before the event/field trip date.
The field trip consent form must be filled out by the field trip coordinator (usually the faculty member) and distributed to students. The forms must then be signed and filled out by each student who will be attending the scheduled field trip. Completed forms must be returned to the Office of Undergraduate Education before the day of the event. Course Fee Funds Requests and Field Trip Consent Forms must be submitted to the OUE office (attn to Paul Cigarruista) or scanned and emailed to cigarrup@uw.edu.
