The BASW Program requires all admitted students to obtain a background check through CastleBranch, an online service.  If you accept this offer, you must initiate the background check within 10-days of receiving this offer letter. Please be aware that there is a $53.00 fee for this service.  Please visit https://portal.castlebranch.com/UQ92 and place your order for ‘UQ92: Background Check’
Acknowledge that you have read and understand that immunization documentation is required for all BASW students.  By signing this form, you acknowledge that the submission of immunization documentation is time sensitive and must be submitted within 10-days.  Note that this is a requirement of the UW Health Sciences Immunization Program (HSIP)—NO EXCEPTIONS ARE MADE. Students submit immunization documentation to CastleBranch, an online service. You will visit http://portal.castlebranch.com/ux50  (different than the background check) to set up your immunization account. You will be charged a one-time setup fee of $43. In addition, the HSIP charges an annual fee of $45 to administer this program which will be billed in quarterly installments to your student account. If documentation is not completed, a hold will be placed on your ability to register for future quarters.

If you were admitted before completing the courses below, you must complete them prior to admission or graduation from the program (consult with academic advisor for completion deadlines):

  • English composition (prior to admission with a minimum 2.0 grade)
  • Introductory course in psychology (prior to graduation with a minimum 2.0 grade)
  • Introductory course in sociology (prior to graduation with a minimum 2.0 grade)

The following requirement must be completed prior to the start of the program

  • Completion of 75 college-level credits

Successful completion of a course in introductory statistics with a minimum grade of 2.0. This requirement may be fulfilled at UW Tacoma.
